Overview

This is the final page of the provider's application process.  It summarizes what has been entered so far, and lets the applicant know what additional steps need to be taken to complete the application process.

Application
- This summarizes the application that is being completed.

Fees - If the applicant owes any money to complete this application, a "Fees" section will appear.  A brief summary of the fees is listed at the bottom of this section, and includes new fees associated with the current application process as well as any unpaid fees from prior applications.

Providers control the acceptable methods of payment.  Options include:

Online payment via credit card.  If the provider allows online payment, you will see options to pay the amount online now, or to pay the amount online later.  Choose the option to pay now unless you are submitting multiple applications one after the other.  If you choose to pay later, you can initiate the payment by clicking the Pay button on the Home Page.  Whether you pay now or later, your payment will be processed using Google Checkout.  When paying online, you will see a line item to cover the electronic processing fees.

Offline payment via check.  If the provider allows offline payment, you will see an option to send a check.   Checks should be made payable to the provider name listed, and sent to the address listed on the page.

Documents - If the provider requires any supporting documentation, and you did not attach an electronic copy of the supporting documentation during your application process, you will see the required documents listed here.  These need to be sent to the address listed on the page.

Terms and Conditions - If the provider requires you to accept any terms and conditions before submitting your application, those terms and conditions will be listed here and you will need to check the box that says "I accept these Terms".  You will be unable to submit your application until you accept the terms.

Save and Continue - Clicking this button will officially submit your application.  The status of your application will change from "Incomplete" to "Under Review".  This does NOT mean that your application has been approved!  Your provider still needs to approve or reject your application. 

If you have chosen the option to pay your fees online now, you will be taken to Google Checkout.  Your credit card has not been charged until you get an e-mail confirmation from Google Checkout, even if you have entered your credit card information and clicked the button to Pay.

You can get to this page clicking the Apply or Renew buttons on the home page, and completing the application.
